The Acceptable Use of Technology Policy (AUTP) policy, which applies to all users logging into any and all church network resources (computers, servers, printers, phone systems, mobile devices, cloud resources and applications), is designed to facilitate the understanding of the expectations for the use of these electronic resources.

The Event Announcement Request form is used to submit events to be announced and displayed on the Cedarville CUMC website, the TVs in the FLC lobby, or Facebook.

Use this online form to request the use of Church buildings and facilities for events.

This is the Rental Agreement Form that would be completed by church trustees and signed by the requestor outlining fee payment and other items. This is provided for reference only to show any requestor the final document to be executed to formalize the usage approval.

The Network Access Form is used to request access for a new user on the CUMC computer network, or to update or terminate an account on the network.

The Payment, or Funds Disbursement Policy explains the process and procedures for requesting payment for an incurred expense. The form to request payment (“Payment Request Form”) is listed below.
